How should it sound?

Make the hurt recognizable to the person who felt it

A useful apology describes what you did and the likely impact in plain language. It does not ask the hurt person to reassure you about your intentions.

Repair is stronger when it is observable. Offer one change you can control rather than promising that nothing difficult will ever happen again.

Questions

What if I did not mean to hurt them?

Intent gives context, but it does not erase impact. Lead with what happened and take responsibility before explaining what you meant.

Can I ask what they need?

Yes, after acknowledging the harm. Do not make them design your entire repair; offer a concrete first step too.
